What is Kyphosis?
Kyphosis is a spinal condition in which an excessive outward curve in the spine causes the upper back i.e. thoracic spine to appear more rounded than normal. All normal individuals have some degree of curvature in their spine but a curve of more than 45 degrees is considered excessive and termed as Kyphosis.
The exaggerated curvature i.e. forward rounding of the back causes a hunched or slouching appearance, commonly referred to as hunchback or round back. Kyphosis can occur at any age and may be congenital, present from birth. It occurs among growing children or adolescents due to malformation of the spine or wedging of the spinal bones over time. The age-related Kyphosis usually occurs due to weakness in the spinal bones that causes them to compress or crack.
Types of Kyphosis
There are a number of reasons and types for the development of Kyphosis such as:
- Postural Kyphosis i.e. poor posture – due to slouching, leaning back in chairs and carrying heavy bags can stretch supporting muscles and ligaments that can increase spinal curvature
- Scheuermann's kyphosis i.e. Abnormally Shaped Vertebrae
- Congenital Kyphosis – occurs due to abnormal development of the spine in the womb causing two or more vertebrae to fuse together.
- Neuromuscular disorders such as Cerebral Palsy, Spina Bifida or Muscular Dystrophy
- Age – as people get older, their spinal curvature can increase, largely due to Osteoporosis
- Degenerative Kyphosis – caused due to spinal deformity by spinal arthritis with degeneration of the discs
- Nutritional Kyphosis- is caused during childhood due to the deficiency of Vitamin D
Symptoms of Kyphosis
Generally, Kyphosis occurs without any symptoms. However, in some cases, symptoms may appear such as:
- Back Pain
- Stiffness or tenderness in the Upper Back
- A Rounded Back
- Tight Hamstrings
- Tiredness
Complications of Kyphosis
Some of the severe cases of Kyphosis may cause complications that include:
- Persistent pain that cannot be controlled with medication
- Breathing difficulties caused by the spine compressing the lungs and airways
In some cases, Kyphosis may compress or pinch the nerves passing through the spine which can disrupt nerve signals and cause symptoms such as:
- Numbness or weakness in the arms and legs
- Problems with a sense of balance
- Loss of normal bladder or bowel control
These serious complications call for urgent treatment, usually surgery.
Treatment of Kyphosis
Kyphosis treatment depends on:
- Patient's Age
- Curve Size
- Symptoms
- The underlying cause of Kyphosis
The Kyphosis treatment approaches are:
- Non-surgical treatment- is recommended for patients with Postural Kyphosis that includes:
- Observation and Monitoring of curve to make sure it does not get worse
- Physical therapy – with specific exercises to help relieve back pain, improve posture and strengthening muscles in the abdomen and back
- Non-Steroidal Anti-Inflammatory Drugs (NSAIDs)
- Bracing - is recommended for patients with Scheuermann's Kyphosis who are still growing
- Surgery:- Surgery is mainly recommended for patients with:
- Congenital Kyphosis.
- Scheuermann's Kyphosis who have curves greater than 75 degrees
- Severe back pain that does not improve with nonsurgical treatment
Spinal fusion is a surgical procedure commonly used to treat Kyphosis. During Spinal Fusion surgery, the surgeon fuses together the affected vertebrae so that they heal into a single, solid bone and reduce the degree of the curve. Due to the fusion of vertebrae, motion between the affected vertebrae is eliminated, which alleviates back pain.
